I fully agree that this should not become a devious way of preventing a hunt or cheating in a PvP, as Sinvaders rightly points out.
As Erisapple notes, it is very difficult to progress in healthcare. But I believe that healthcare is part of active RP (I insist on active), as Eolinius says perhaps better than me. Let's not forget that the characteristic of Ryzom is that it does not have "classes".
I also find very clever your different ideas of "control" of abuse imagined by Haokan and Magez. It doesn't seem illogical to me to lose fame based on who was treated under what circumstance, as well as having a recharge timing to avoid abuse.
Personally, when there are mass actions, all confused in a common activity, I often use help on those who are in the crowd, without looking for who is who. I lift up the one who is far away, recharge the self in sap the tired healer, and so on...
So many things can be done, helping to progress in the "healer class" :)
