It is not default icon, the icon of your action depends on configuration. It seems to me that the highest "rank" action (action with highest ID) icon is used. It applies to every action, not just forage ones.
Hmm, maybe trainers could sell very high ID actions named "Icon" that one could buy to change the icon...
