One thing I can suggest is that you are somehow missing an outlier member of the camp. Given the thoroughness that you have displayed, I think it unlikely, but that's my thought.
The other is that somehow you are running 'out of bounds' during your extirmination and that is canceling the trigger. The mission for the diseased Gingo in the Jungle doesn't trigger if you run out of the canyon, even if you kill the Gingo. If that is the case, the only solution is going to be abandoning the mission and starting over, which is a serious pain in the arse.
If you can get a high level CSR on the line, they may be able to change your variables so that the mission step is properly recorded.
